Phoenix Home Care & Hospice in Kansas City, MO is seeking an Admissions RN to serve as the first point of contact between patients and hospital teams. You will assess patients, interpret diagnostics, and develop individualized plans of care in collaboration with physicians and therapists.

Qualificações

• Missouri or Compact Nursing License.
• Strong organizational and self-management skills.
• Valid driver’s license.

Responsabilidades

• Skilled visits to patients.
• Day-to-day patient interaction.
• Education on health conditions (e.g., CHF, COPD, Diabetes).
• Wound care and/ or dressing changes as prescribed.
• Medication management and monitoring.
• Documentation of patient status and changes for physician review.
• Collaborate with nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist.

• Pay Range: $75,000 - $90,000

As a Phoenix Admissions RN, you will be the first point of contact between a patient and hospital. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

Responsibilities:

• Skilled Visits
• Day-to-day patient interaction
• Education on Health Conditions, such as CHF, CODP, Diabetes
• Wound Care
• Medication Management
• Changes dressings, bandages and contraptions as prescribed
• Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.
• Working alongside other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist.
