Job summary

Adoration Health is seeking a passionate Home Health Registered Nurse to join our team in San Antonio, TX. In this role you will provide expert, patient-centered home health care and collaborate with an interdisciplinary team to support patients and families.

You will assess patients, develop care plans, administer medications, and educate families while working in a supportive environment with regular on-call rotation and opportunities for advancement.

Qualifications

• RN license in state of practice required.
• One year nursing practice in patient care setting; home health/geriatrics preferred.
• Valid driver's license and proof of car insurance.
• New graduates may be considered in select markets.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligning with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required


• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred


• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy


• New nursing graduates may be considered in select markets based on program availability


• Current CPR certification
