Job summary

Allay Hospice in Wisconsin is seeking a dedicated Hospice RN Case Manager to deliver patient-centered care within a supportive, team-oriented environment. You will be responsible for assessments, care planning, and providing direct nursing care across a full-time schedule, Monday–Friday 8 AM–5 PM.

Qualifications

• Graduate of an accredited nursing program with current RN license in Wisconsin.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or home health setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administer medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
