Job summary

Allay Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Wisconsin. This role offers lower patient-to-nurse ratios, strong interdisciplinary support, and leadership that understands hospice nursing.

Qualifications

• Graduate of an accredited nursing school with current RN licensure in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of hospice philosophy of care; commitment to clinical and documentation excellence.
• Must have a valid driver’s license with reliable vehicle.

Responsibilities

• Assess, monitor, and address physical, emotional, and psychosocial needs of patients.
• Create individualized hospice care plans aligned with patient goals.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management and comfort care.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by local branch.

Responsibilities

As a RN Case Manager You will:

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
