Job summary

Allay Hospice is seeking a Hospice RN Case Manager to provide compassionate, one-to-one patient care across Milwaukee-area counties. You will assess needs, develop personalized care plans, and coordinate direct nursing services including medication administration and symptom control.

The role offers strong interdisciplinary support and a stable, growth-oriented environment. Requirements include an active RN license and 1 year of nursing experience, with hospice familiarity preferred.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the state of operation.
• 1 year of nursing experience required, 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of the hospice philosophy of care.
• Must have a valid driver’s license and insured vehicle for state requirements.

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care including administering medications, treatments, and interventions.

Hospice RN Case Manager RN — Where Support Meets Impact

Office Location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045Coverage area:Travel throughout the territory is required- Primarily Waukesha and Milwaukee Counties

Schedule:Full Time Monday-Friday 8AM-5PM

  Qualifications

  • Commitment to clinical and documentation excellence
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
