Job summary

Adoration Health is seeking a dedicated RN Case Manager for hospice in the Greenwood, IN area. You will manage patient care plans, administer nursing care, and coordinate with an interdisciplinary team to support patients and families.

Join a supportive, patient-centered environment with potential for growth and on-call rotation as needed. A valid RN license and CPR certification are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with a vehicle insured as required.
• Maintain CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
