Job summary

Hospice of North Alabama is seeking a compassionate RN Case Manager to join our Decatur-area team. You will assess patient needs, coordinate care plans, and provide expert symptom management in collaboration with an interdisciplinary group.

This full-time role offers a Monday–Friday schedule in a supportive environment, opportunities for growth, and the chance to make a meaningful impact on patients and families during a difficult time.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess / monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
