Job summary

Hospice of North Alabama is seeking a dedicated Registered Nurse Case Manager to join our team in Cullman, AL. You will assess patients’ physical, emotional, and psychological needs and develop individualized hospice care plans aligned with patient goals.

Responsibilities include direct nursing care, pain management, educating families, collaborating with an interdisciplinary team, maintaining meticulous documentation, and participation in on-call shifts.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order
