Job summary

BJC Home Care is seeking an RN who will act as a liaison among patients, physicians, hospital staff and home health teams to coordinate planning for home care and support discharge planning.

On-site role primarily at Barnes Jewish West County Hospital with weekend and holiday rotations, travel between locations, and mileage reimbursement. Multitasking and prioritization are essential to success.

Qualifications

• Nursing diploma or associate's degree in nursing.
• RN license or equivalent required.
• Experience coordinating care and discharge planning is preferred.

Responsibilities

• Contributes to discharge planning in collaboration with health care providers.
• Documents all coordination of care activities.
• Participates in planning home health care services with health care staff.
• Educates hospital staff and community on home care and specialty services.
