Archway Transitional Care in Macon, GA is seeking an experienced RN to lead assessments, develop care plans and coordinate interdisciplinary care for residents. You will conduct patient and family interviews, review medical data, and ensure plans reflect patient wishes and regulatory standards.

The role emphasizes collaboration with the care team, supervision of the RAI Coordinator, and ongoing staff education on MDS processes to maintain high-quality, patient-centered care in a nonprofit

Qualifications

• Nursing degree from an accredited college or university.
• Active RN license to practice in Georgia.
• Proficient in MS Office and healthcare software.

Responsabilités

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with interdisciplinary team members to provide a comprehensive and holistic patient care plan.
• Coordinates the development of a care plan for each patient with problems, interventions and measurable goals.
• Oversees MDS process accuracy and educates staff on MDS requirements.
