The ideal candidate holds a GA RN license, a nursing degree, and strong communication, coordination, and documentation skills to develop and update patient care plans in a collaborative, mission-driven, non-profit setting.

Qualifications

• Nursing degree and GA RN license.
• Experience in long-term care preferred.
• Strong communication and documentation skills.

Responsibilities

• Coordinate patient assessments and care plans.
• Communicate with interdisciplinary team members.
• Educate staff on care plans and MDS process.
• Monitor accuracy of MDS process and care plan documentation.
• Assist in management reports and special assignments.
