Job summary

Shalom Gardens Health & Rehab in Richmond, VA is seeking a Registered Nurse (RN) to serve as QA, SDC, and IP. This dual-role leadership position focuses on quality improvement, staff education, and infection prevention while ensuring compliance with CMS, state regulations, and facility policies.

The RN will coordinate orientation, deliver ongoing training, track competency, mentor nursing staff, and lead audits and action plans to drive clinical excellence and resident outcomes.

Qualifications

• Active RN license in Virginia.
• At least 2 years nursing experience (long-term care/SNF preferred).
• Experience in infection prevention, staff education, or quality assurance.
• Knowledge of CMS regulations, QAPI, and infection control standards.
• Strong organizational, leadership and communication skills.
• Ability to analyze data and drive process improvements.

Responsibilities

• Monitor clinical outcomes, quality indicators, and performance metrics.
• Lead QAPI initiatives and audits for regulatory compliance.
• Coordinate orientation and ongoing education for new hires.
• Develop training programs and maintain competency records.
• Oversee infection prevention and control programs; track infections per CDC/CMS.
• Educate staff on infection control practices and lead audits.
