Job summary

UW Health in Madison, WI seeks a Registered Nurse to coordinate care for Home-Based Hospital Care (HBHC) patients via virtual visits and phone contacts, ensuring hospital-level care in the home. The role is hybrid with onsite work, remote work, and travel within the Madison area.

You will collaborate with rounding providers, equipment coordinators, admission RNs, and in-home staff to support planning, monitoring, and safe discharge. Strong documentation in EPIC is required.

Qualifications

• Bachelor's Degree in Nursing (BSN) preferred.
• 2 years of relevant acute care or home health nursing experience required.
• Experience in care coordination, triage, or nursing leadership preferred.
• Experience in program development preferred.
• Experience in teaching self-care skills to patients and families preferred.
• Recent, relevant home health experience preferred.
• Experience using EPIC and virtual health platforms preferred.
• Experience with IV therapy and knowledge of venous-access devices and ambulatory pumps preferred.

Responsibilities

• Coordinate care for HBHC patients as the central contact through virtual visits and phone communication.
• Partner with rounding provider, equipment coordinator, admission RN, and in-home RNs/paramedics to support daily care planning and safe transitions of care.
• Conduct virtual medication administration visits and ongoing virtual patient check-ins with clinical oversight.
• Participate in interdisciplinary rounds and manage post-acute referrals, discharge services, and equipment coordination.
• Maintain thorough documentation in the electronic medical record and monitor patient progress.
