Job summary

UW Health is seeking a Registered Nurse (RN) Admission for a nursing role in Madison, Wisconsin. The position involves on-site work at University Hospital and East Madison Hospital with some travel between sites, focusing on enrolling patients into the Home-Based Hospital Care (HBHC) program.

The role emphasizes autonomy and collaboration with the HBHC team, with a schedule of day/evening shifts and weekend/holiday rotation.

Qualifications

2 years of relevant acute care or home health nursing experience required.
Care coordination, triage, or nursing leadership experience preferred.
Experience in program development preferred.
Experience teaching self-care skills to patients and families preferred.
Recent home health experience preferred.
Experience using EPIC and virtual health platforms preferred.
Experience with IV therapy and venous-access devices preferred.

Responsibilities

Conduct bedside intake, eligibility screening, and informed consent discussions for home-based care.
Partner with admitting physician to review patient lists and coordinate timely discharges to HBHC.
Prepare patients for departure by arranging transportation and supplies, ensuring smooth handoff to HBHC team.
Collaborate with hospital and home-based teams to launch each HBHC episode successfully.
