Job summary

Logan Health in Polson, MT seeks an RN Primary Care Navigator to connect patients with essential services. You’ll focus on chronic disease management, care coordination, education, and clinic support, serving a wide age range from pediatrics to elderly.

Location is Polson Primary Care Clinic; 40 hours/week, day shift. Strong communication and independent work style are essential; prior navigation experience is a plus.

Qualifications

• Current Montana RN license or multi-state compact allowed, required.
• Bachelor’s Degree in Nursing preferred.
• RQI Provider (BLS) within 7 days of hire/transfer.
• Knowledge of electronic health records.
• Autonomy and strong communication skills.
• Experience in care coordination or navigation preferred.

Responsibilities

• Enroll patients in CCM program and educate on program benefits.
• Coordinate care across primary care, specialty, inpatient and outpatient settings.
• Facilitate scheduling of appointments, referrals, testing and procedures.
• Educate patients on chronic disease management, medications and treatment plans.
• Support transitions of care and ED follow-up.
• Track patient progress and follow up to close care gaps.
