Job summary

Logan Health in Polson, MT is seeking an experienced RN to join our Primary Care Navigator team. You will connect patients with essential services, focus on chronic disease management, care coordination, ED follow-up, and clinic support to ensure smooth transitions and continuous care.

Qualifications

• Montana RN license is required or multi-state compact for MT eligibility.
• BLS/RSI provider within 7 days of hire.
• Proficient computer skills; experience with EHR systems preferred.
• Ability to work autonomously and collaborate with multidisciplinary teams.
• Experience in care coordination and understanding of care transitions.
• Knowledge of value-based care models and chronic disease management is preferred.

Responsibilities

• Enroll patients in CCM program and educate on benefits.
• Outreach to patients needing navigation services.
• Educate patients on chronic disease management, meds, and treatment plans.
• Coordinate care across primary, specialty, inpatient, and outpatient settings.
• Schedule appointments, referrals, tests, and procedures in a timely manner.
• Assist patients in accessing housing, care assistance, and placement resources.
• Engage with providers to optimize patient outcomes and communications.
• Track patient progress and perform follow-up to ensure ongoing care.
