Job summary

Cape Fear Valley Health System seeks a Care Advocate to serve pregnant and postpartum patients from enrollment through 12 months postpartum. You will perform clinical assessments, coordinate care, and educate on self-management and newborn care.

The role requires 2 years obstetrics/maternal care experience and NC RN licensure. Strong communication, trauma-informed approach, and collaboration with OB/ pediatrics teams are essential.

Qualifications

• Associate Degree in Nursing required.
• Current RN licensure in North Carolina or Compact state licensure required.
• 2 years obstetrics, maternal/newborn care, family medicine or pediatrics required.
• 1 year in care coordination, triage, patient education, or case management preferred.
• 1 year of mental health/SUD experience preferred.

Responsibilities

• Review automated screening results and relevant EHR information daily.
• Conduct clinical assessments within licensure/scope related to symptoms, follow-up, postpartum concerns, and risk monitoring.
• Provide education on prenatal/postpartum self-management and newborn care basics.
• Coordinate communication across OB, pediatrics, primary care, behavioral health, and specialties.
• Triage medical issues and escalate behavioral health/SUD risks per SOPs.
• Document assessments, education, plans, and follow-ups in the program platform and EHR.
• Maintain logs and dashboards for fidelity and monitoring.
• Collaborate with clinic staff and community partners to support whole-person care.
