Job summary

Cape Fear Valley Health System is seeking a Care Advocate in Fayetteville, NC. This role supports pregnant and postpartum patients from enrollment to 12 months postpartum with empathetic engagement, clinical assessment within scope, education, and proactive care coordination across OB, pediatrics, primary care, and behavioral health.

Qualifications

• Associates Degree in Nursing is required.
• Bachelor's Degree in Nursing is preferred.
• Current RN licensure in North Carolina or Compact state licensure required.

Responsibilities

• Review screening results and EHR data daily.
• Conduct clinical assessments within licensure scope.
• Provide prenatal/postpartum education and care coordination.
• Reinforce provider plans and coordinate follow-up appointments.
• Triage medical issues and escalate behavioral health concerns as needed.
• Complete warm handoffs and schedule referrals.
• Document assessments and plans in the program platform per site standards.
• Communicate with clinical teams about medical concerns and risk changes.
• Maintain logs and dashboards for fidelity and monitoring.
• Collaborate with partners to support whole-person care.
• Participate in onboarding and ongoing training.
