Job summary

Qualifications

• Associates Degree in Nursing required; Bachelor’s preferred.
• Current NC RN licensure (or Compact state).
• 2 years obstetrics/maternal-newborn or pediatrics experience required.
• Experience with care coordination, triage nursing, patient education, or case management preferred.

Responsibilities

• Review automated screening results and relevant EHR information daily.
• Conduct clinical assessments within licensure/scope of practice.
• Educate on prenatal/postpartum self-management, warning signs, lactation and medications as appropriate.
• Reinforce treatment plans and ensure follow-up appointments.
• Coordinate communication across OB, pediatrics, primary care, behavioral health, and specialties.
• Triage medical issues and escalate behavioral health/SUD/social risk per SOPs.
• Document assessments, plans, and follow-ups in the program platform and EHR.
• Maintain logs and dashboards for fidelity and monitoring.
• Collaborate with team members and community partners for whole-person care.
• Participate in onboarding, training, supervision, and learning activities.
