Job summary

PureHealth Transitional Care at THR Arlington seeks a licensed practical nurse (LPN) to render nursing care in a post-acute setting. The role reports to the Unit Manager and follows physician orders within the scope of practice.

The position offers full-time hours with 6a-6p shifts and PRN coverage to ensure continuous patient care. You will document care in the EMR, participate in care planning, and collaborate with an interdisciplinary team to meet patient needs.

Qualifications

• High School Diploma or Equivalent.
• Graduate of an accredited LPN program.
• Valid State Appropriate LPN license.
• Current CPR certification.

Responsibilities

• Provide nursing care in assigned area per standards and nursing function scopes.
• Document observations and interventions in EMR.
• Administer medications accurately and monitor patient responses.
• Lead team conferences to develop individualized care plans.
• Coordinate communication among patient, family, and healthcare team.
