Job summary

Kingman Regional Medical Center in Kingman, AZ, seeks a Population Health Program Manager (RN required) to lead population health initiatives aligned with our strategy. This on-site role reports to the Director of Practice Operations and oversees program development, performance management, and care transformation through partnerships with clinicians and payers.

Responsibilities include steering committees, analytics-driven reporting, and cross-department coordination to ensure timely delivery

Qualifications

• Education: Bachelor's degree in Population Health, Clinical Quality/Clinical Process Improvement, or Nursing.
• Licensure: Current, valid AZ RN License.
• Experience: 3 years in population health, quality/process improvement, nursing leadership, or project management.
• Skills/Knowledge: Excellent communication and presentation; work with clinicians and staff.
• Demonstrated project management and organizational skills.
• Robust analytics and problem-solving abilities.

Responsibilities

• Supports strategic planning for evolving population health plans and monitors KPIs.
• Leads content for key committees overseeing performance and projects.
• Coordinates with analytics to develop reporting tools for actionable insights.
• Acts as a care transformation agent through strong partnerships with providers.
• Forges physician partnerships with open communication and trust.
• Collaborates with operations, analytics, and clinical leadership across population health.
