Job summary
Kingman Regional Medical Center (KRMC) is seeking a Population Health Program Manager (RN required) to lead the development, implementation, and performance management of population health initiatives. This on-site role reports to the Director of Practice Operations and offers strong growth opportunities within a large rural health system.
The ideal candidate has a nursing license and 3 years of population health, quality improvement, or nursing leadership experience.
Qualifications
- Education: Bachelor's degree in Population Health, Clinical Quality/Process Improvement, or Nursing.
- Licensure: Current, valid AZ RN License.
- Experience: 3 years in population health, quality/process improvement, nursing leadership, or project management.
- Skills/Knowledge: Excellent communication and presentation; effective with clinicians and staff; strong analytics and project management.
Responsibilities
- Supports strategic planning for evolving population health plans aligned with organizational priorities and monitors KPIs.
- Leads and develops content for committees overseeing performance and projects in the population health portfolio.
- Coordinates with analytics to develop reporting tools and actionable reports.
- Acts as a care transformation change agent through partnerships with providers.
- Fosters physician partnerships with a shared vision and open communication and trust.
- Collaborates with operations, analytics, and clinical leadership across population health operations.
- Supervises nurse navigators in outpatient clinics.
- Performs other duties to support department and organization.
