Qualifications

Responsibilities

• Conduct comprehensive home assessments for patients.
• Provide training and education on CAPD and APD.
• Monitor monthly laboratory trends and manage catheter care.
• Collaborate with nephrologists and multidisciplinary teams to adjust treatment plans.
• Maintain accurate, detailed reports and records.
• Administer medications and monitor patients for reactions or side effects.
• Record patients' medical information and vital signs.
• Consult and coordinate with healthcare team members to evaluate patient care plans.
• Modify patient treatment plans based on responses and conditions.
