Job Summary

Full time, 40 hours, remote call rotation required

We are currently seeking an experienced peritoneal dialysis RN interested in becoming part of a team dedicated to empowering patients with end-stage renal disease (ESRD) to safely manage their treatments at home. Operating primarily on a Monday-through-Friday schedule, the PD RN acts as a patient educator, clinical evaluator, and coordinator of care. Key responsibilities include conducting comprehensive home assessments, conducting training, and teaching patients and their families how to perform manual (CAPD) and automated (APD) peritoneal dialysis using strict sterile techniques to prevent peritonitis. Additionally, the nurse provides longitudinal clinical oversight by conducting regular outpatient clinic visits, monitoring monthly laboratory trends, managing catheter care, and collaborating with nephrologists and multidisciplinary teams to adjust treatment plans and maximize patient independence.

Summary

Accountable for interpreting the plan of medical care, assessment of patients' clinical decision-making regarding nursing care, assuring nursing care is provided in a safe and competent manner, providing individualized nursing care, and evaluating nursing care for groups of patients.
