Responsibilities

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

• Provide high-quality clinical services within scope of practice and infection control standards, following the care plan, and coordinating with the care team from admission to discharge.
• Complete clinical assessments as per federal/state requirements and payer guidelines.
• Verify patient eligibility and medical necessity for services.
• Develop and revise individualized care plans according to regulatory requirements and in collaboration with community providers.
• Ensure care plans meet patient needs, initiating revisions with physician approval as necessary.
