Responsibilities

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

• Provides high-quality clinical services within scope of practice and infection control standards, in accordance with the care plan and in coordination with the care team from admission to discharge.
• Completes clinical nursing assessments as per federal and state requirements and payer specifications.
• Ensures patient eligibility and medical necessity for services according to payer and agency policies.
• Develops and revises individualized care and service plans according to federal and state requirements, collaborating with community providers.
• Ensures the care plan's frequency and duration meet patient needs and revises plans as needed with physician approval.
