This is a PRN position.

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and policies. The RN coordinates the care plan with other healthcare professionals and helps ensure continuity of care by planning and exchanging information with physicians, agency staff, patients, families, case managers, and community resources.

• Provides high-quality clinical services within scope of practice, infection control standards, and in accordance with the care plan from admit to discharge.
• Completes clinical nursing assessments as required by federal and state programs and payers.
• Ensures patient eligibility and medical necessity for services.
• Develops and revises individualized care plans with other providers.
• Ensures care plan meets patient needs and revises as needed with physician approval.
