Qualifications

• Current RN license or equivalent licensure (EMT/Paramedic accepted).
• 2–3 years of clinical nursing experience.
• Critical care, emergency, or medical-surgical experience preferred.
• Strong communication and interpersonal skills.
• Experience with electronic health records and transfer systems.

Responsibilities

• Coordinate inter-facility transfers across the care continuum.
• Perform clinical assessments to determine level of care.
• Communicate with physicians, nurses, and transfer teams.
• Manage multiple phone calls to minimize delays.
• Document transfer details in EHR systems.
• Coordinate transportation arrangements (ground/air).
• Monitor bed availability with bed management teams.
• Serve as clinical resource for transfer questions.
• Participate in quality improvement of transfer processes.
• Utilize emergency notification systems for STEMI/stroke/trauma transfers.
• Proactively identify and escalate potential issues.
• Prepare reports and workload summaries as requested.
