Job summary

CHRISTUS Health seeks a Registered Nurse for the Transfer Center to coordinate and document patient transfers into the CHRISTUS Health System. You will collaborate with referring physicians, ED and inpatient staff, and hospital leadership to ensure safe and timely transports with appropriate levels of care.

The role requires an ADN, active Louisiana RN license or compact licensure, and BLS, with emphasis on customer service and EMTALA awareness, supported by Meditech and transfer software

Qualifications

• Associate's Degree in Nursing is required.
• RN License in state of employment or compact licensure.
• Basic Life Support (BLS) certification required.

Responsibilities

• Evaluate, coordinate, and document patient transfers into CHRISTUS Health System.
• Collaborate with physicians, case managers, and hospital staff to ensure appropriate level of care and transport.
• Maintain accurate medical and demographic information and ensure EMTALA compliance.
• Communicate with referring and receiving facilities and monitor bed availability.
• Maintain professionalism and patient-centered service while adapting to transfer priorities.
