Responsibilities

• Patient Transfer Coordination: facilitate safe transfers for emergent/complex cases with physicians and transport staff
• Clinical Assessment: perform triage to determine level of care and subspecialty needs for transfer decisions
• Communication: act as liaison between referring and receiving facilities and document transfer requests
• Interprofessional Collaboration: work with interdisciplinary teams for patient-centered care
• Documentation: record all transfer activities in EMR and transfer center software
• Quality & Safety Improvement: participate in quality improvement activities and incident debriefings
