Job summary

Atrium Health University City - Case Management is seeking a Full-time Registered Nurse Case Manager to coordinate patient care, reduce barriers, and ensure timely delivery of services. 40 hours per week, Mon-Fri 0800-1630, on-site in Charlotte, NC.

You will work with multidisciplinary teams, manage a caseload, assess patient needs, and navigate contracts and resources to support transitions of care. Requires RN license and typically 5 years nursing or 1–2 years care management experience, Epic

Qualifications

• Registered Nurse license required in the state of practice.
• Bachelor's Degree in Nursing or related field.
• Typically 5 years of clinical nursing experience or 1–2 years in care management.

Responsibilities

• Facilitates communication among patient/family and multidisciplinary teams to identify and resolve barriers to care.
• Collaborates with primary care, hospital care teams, and post-acute managers to ensure quality, efficient care.
• Uses evidence-based approaches to increase patient activation and engagement in care.
• Identifies learning barriers and reports findings to the care team to plan next steps.
• Independently manages a CM caseload and ensures timely documentation for regulatory requirements.
• Coordinates transitions of care and referrals to internal or external resources.
