Responsibilities

• Coordinate patient transfers for emergent/urgent/complex cases.
• Perform triage assessments to determine level of care for transfers.
• Act as liaison between referring and receiving facilities; document requests.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Document transfer activities in EMR and transfer center software; generate monthly reports.
• Participate in quality and safety improvement activities related to transfers.
