Job summary

UTMB Health seeks a Patient Navigator (RN) at Sealy Heart and Vascular Institute to guide patients through the care journey. You will explain procedures, prep, recovery and follow up, and coordinate with families and referring physicians to ensure smooth care delivery.

Qualifications

• Bachelor’s degree in Nursing.
• Three (3) years of RN nursing or RN patient navigator experience.
• Current RN license or valid permit to practice nursing in Texas and BLS.

Responsibilities

• Guide patients through the healthcare process and coordinate care.
• Communicate with patients and families about procedures, preparation, recovery and follow up.
• Liaise with referring physicians to ensure seamless care and address patient concerns.
• Help reduce patient anxiety by clearly explaining procedures and answering questions.
