LCMC Health in New Orleans, LA seeks an experienced Registered Nurse to coordinate patient care plans and support families through treatment decisions. This role connects patients with physicians, nurses, social workers, and other caregivers to ensure seamless care.

Responsibilities include patient education, care coordination across inpatient and outpatient services, and contributing to policy updates and staff education as needed.

Formación

• Licensed RN in Louisiana or eligibility.
• BSN preferred or able to obtain within 3 years of hire.

Responsabilidades

• Coordinate written and verbal communication of treatment plans among patient, family, physicians, and care team.
• Coordinate treatment plan communication with internal/external departments to ensure continuity of care.
