Job summary

Qualifications

• BSN required or must obtain within three years of hire.
• Louisiana RN license is required.
• American Heart Association BLS certification is required.

Responsibilities

• Coordinate written and verbal communication among patient, family, physician, staff, and team members.
• Communicate and coordinate treatment plans to internal and external departments to ensure continuity of care.
• Assist patients in understanding diagnosis, treatment options, and available resources.
• Review medical documentation to aid physicians in obtaining complete treatment plans.
• Serve as essential link between patients and care providers.
• Arrange appointments for consults and support services within service standards.
• Monitor patient throughout care continuum, including inpatient and care management coordination.
• Collect data, track outcomes, and support planning with service line personnel.
• Provide nursing care and guidance from screening to survivorship.
• Educate patients and families; document teaching and use appropriate education modalities.
