Northwest Medical Center in Tucson, AZ is seeking a Patient Navigator - RN for Structural Heart in the Cath Lab. This full-time role provides guidance, supports patients and families, coordinates care, and ensures clear communication across teams.

The navigator will assess needs, educate patients, assist with insurance matters, and help navigate referrals and resources throughout treatment from diagnosis to completion.

Qualifications

• 3–5 years of clinical nursing experience.
• 1–3 years of experience in patient navigation, care coordination, or case management preferred.
• RN licensure and BCLS/BLS certification required.
• Familiarity with insurance processes and patient education.

Responsabilités

• Serve as primary point of contact for patients, families, and caregivers from diagnosis through end of treatment.
• Assess medical, emotional, and socioeconomic needs; address barriers to care and coordinate solutions.
• Provide patient education on treatment, side effects, and management strategies.
• Facilitate communication among patients, caregivers, and healthcare providers to share information.
• Coordinate referrals to social workers, financial counselors, dietitians, and community resources.
