Job summary

Qualifications

• Requires knowledge of nursing assessment, planning, implementation and evaluation.
• RN licensure in Ohio; BLS required; ACLS within 6 months where applicable.

Responsibilities

• Assess patient and family needs and safety risks; develop and revise care plan.
• Communicate plan using SBAR; participate in shift reports and care conferences.
• Document care in the EMR accurately and timely; ensure handoffs between teams.
• Provide discharge planning and referrals as appropriate.
• Respond to emergencies with decisive, coordinated action.
