Job summary

Signature HealthCARE in Erin, TN is seeking compassionate RN candidates to provide direct nursing care to residents, administer medications, and support a rehab-to-home program. The role emphasizes collaboration with an interdisciplinary team to maintain high-quality, resident-centered care.

Ideal applicants hold an active RN license, have at least 1 year of clinical nursing experience, and possess strong documentation and EHR skills.

Qualifications

• Current, active RN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role is preferred.
• CPR/BLS certification preferred.
• Familiarity with electronic health records (EHR) systems and basic MS Office usage.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Coordinate admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
