Job summary

Qualifications

• Specialized Diploma from an accredited nursing program.
• Cultural awareness and ability to convey respect to diverse patients.
• Effective communication and collaboration with patients, families and the care team.
• Knowledge of discharge planning and patient education.
• Experience in an acute care setting preferred.
• Active RN license required in the state of practice (PA).
• BLS certification required; AHA basic life support acknowledgement.

Responsibilities

• Interacts with patients, families, and staff to deliver patient-centered care.
• Integrates evidence-based practice and clinical expertise in care delivery.
• Educates patients and families about conditions, treatments and self-care.
• Uses health IT to document, communicate, and monitor care outcomes.
