Yale New Haven Health is seeking a Registered Nurse (RN) to join our patient‑centered care team in Connecticut. The RN will work within the Connecticut Nurse Practice Act and ANA standards to provide high-quality nursing care guided by our mission, vision, and values.

The role emphasizes autonomy, accountability, collaboration, and leadership in daily practice, coordinating care across the patient and family journey while ensuring safety and quality outcomes.

Qualificações

• Graduation from an accredited school of nursing.
• BSN preferred.
• All newly hired registered nurses must have current BCL.

Responsabilidades

• Assessment: Collects pertinent data and identifies health concerns.
• Outcomes Identification: Sets expected outcomes for a care plan.
• Planning: Develops and updates a plan of care with strategies.
• Implementation: Delivers the care plan and coordinates actions.
• Coordination of Care: Manages transitions between care settings.
• Health Teaching and Health Promotion: Educates patients and families on health strategies.
• Evaluation: Reviews progress and outcomes of care.
