This full-time, day shift role involves rotation between inpatient and outpatient care at Inova Fairfax Hospital and the Inova Center for Personalized Health, near the main campus.

Qualifications

• Bachelor's Degree in Nursing is required.
• 3 years of nursing experience.
• Licensed in Virginia as a Registered Nurse and BLS certified.

Responsibilities

• Guide patient and family through diagnosis, testing, treatment and follow-up care.
• Coordinate plan of care with Transplant Infectious Disease team and schedule tests.
• Educate patient on treatment options and assess knowledge for complex regimens.
• Assist with accessing resources and support services.
• Collaborate with multidisciplinary teams to address questions about care.
• Contribute to improving quality outcomes such as reduced infection rates and readmissions.
• Develop and maintain reliable systems to document and monitor patient data.
