Talent With Heart is seeking a dedicated Patient Care Manager (RN) to lead our home health team and elevate patient care in the community. You will guide clinical staff, foster collaboration, and ensure safety, trust, and clinical quality across services.

As a clinical leader, you will influence patient outcomes, support staff development, and uphold high standards of regulatory compliance in a compassionate work environment.

Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and staff development.
• Facilitate effective communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
