Soap Creek Resources in California seeks a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. You will guide clinical staff to foster a collaborative, safe environment where care teams thrive and deliver compassionate services.

As a clinical leader, you will improve operational excellence, ensure regulatory compliance, and influence patient outcomes while supporting growth for your team and showcasing organizational values.

Formación

• RN license in the practicing state (California).
• Minimum 5 years of RN experience, including home care.
• At least 2 years of management or supervisory experience in home health preferred.
• Knowledge of Medicare Conditions of Participation and applicable regulations.

Responsabilidades

• Provide clinical supervision to ensure patient care aligns with professional standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and organizational values.
• Maintain rigorous compliance through training, documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing safety protocols.

Responsibilities:

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.

Qualifications:

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
