Qualifications

• RN with current licensure in the practicing state.
• Minimum 5 years of RN experience, with home care experience.
• 2 years of management or supervisory experience in home health preferred.
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulations.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
