Job summary

Eden Capital Careers is seeking a Patient Care Manager (RN) in California to lead our home health team and elevate patient care across the community. As a clinical leader, you will empower and guide staff, foster collaboration, and drive excellence in compassionate home health services.

You will champion operational excellence, regulatory compliance, and clear communication to influence patient outcomes, support professional growth, and uphold safety and clinical quality across the organization.

Qualifications

• Active RN license in California.
• Minimum 5 years RN experience, home care preferred.
• 2 years supervisory experience in home health.
• Knowledge of Medicare Conditions of Participation and regulations.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.

Qualifications:

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
