Qualifications

• Registered nurse with current licensure to practice in the state.
• Minimum 5 years of RN experience, preferably in home health.
• At least 2 years of supervisory or management experience in home health.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.

• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
