Job summary

Top Dog Recruiting is seeking a Patient Care Manager (RN) to lead our home health team in Newport Beach. You will supervise clinical staff, ensure compliance, and drive high-quality patient outcomes with safety and compassion at the core.

You will champion operational excellence, mentor staff, and partner with families and referral sources to maintain service excellence and regulatory adherence in a dynamic home health setting.

Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements

Responsibilities

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.
