Job summary

You will supervise clinical personnel, drive quality initiatives, and collaborate with families, referral sources, and care teams to achieve optimal patient outcomes.

Qualifications

• Active RN license in the practicing state.
• Minimum 5 years RN experience, preferably in home care.
• 2+ years management or supervision in home health (preferred).
• Knowledge of Medicare Conditions of Participation and federal/state regulations.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and ongoing staff development.

Qualifications:

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
