H.T. Prof Executive Search is seeking a Patient Care Manager (RN) to lead our home health team in Culver City. In this role you will supervise clinical staff, drive patient care quality, ensure regulatory compliance, and foster a collaborative, compassionate care environment.

You will champion safety, coordinate care plans, coach staff, and implement quality improvement initiatives to meet performance goals while upholding agency policies and Medicare Conditions of Participation.

Qualifikationen

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 years RN experience with prior home care experience.
• 2 years of management or supervisory experience in home health.
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulatory requirements.
