Modern Machining Solutions Inc. seeks a proactive Patient Care Manager (RN) to lead our home health team and elevate patient outcomes across the community.

You will guide clinical staff, ensure regulatory compliance, drive quality improvements, and model professional leadership in a patient-centered, safety-focused environment. Collaborating with families, referral sources, and cross-functional colleagues, you will promote excellence and staff development.

Competenze

• Registered nurse with current licensure to practice in the state.
• Two years management or supervisory experience in home care preferred.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Position Overview

As a Patient Care Manager (RN), you play a pivotal role in leading our home health team and elevating patient care across the community. • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
